Every year, we reach nearly 10,000 seniors throughout 13 counties with our services.
Meals on Wheels (MOW) Social Work Team provides a safety net for seniors who are at an increased risk of abuse, neglect and exploitation. Our Social Workers link resources to seniors allowing them to remain independent in their chosen living environment. The Meals on Wheels Community-Based Social Worker is a full-time, Monday - Friday days only position in which the Community-Based Social Worker will work with the Case Management Department and is responsible for developing needs assessments and care plans for our senior client population.
Medical, dental and vision benefits available day one. Ten paid holidays per year and paid time off accrual starts day one. Starting salary is based on experience.
PRIMARY FUNCTION: Serve as a professional member of a multidisciplinary team, sharing in the responsibility of developing needs assessments and care plans for our senior client population. Will also serve as a social worker for those seniors who need assistance in remaining independent, working closely with individual clients to ensure their needs are being met by assisting in budget planning, paying bills and money management counseling.
TYPICAL WORK CONDITIONS: Work is performed remotely.
ESSENTIAL JOB FUNCTIONS:
- Responsible for an assessment, development and implementation of a comprehensive service plan and monitoring in consultation with the Manager. The duties and responsibilities may include completing a thorough assessment of client situations to determine presence and degree of risk for abuse, neglect or exploitation for assigned client.
- In collaboration with client, caretakers, and community partners if appropriate, develops or follows a COA developed plan of care to address the identified needs of the senior. Implements and monitors (for reasonable time) care plan and delivery of services.
- Establishes and maintains a positive working relationship with client, their caretaker, and community partners (if applicable) encouraging them to utilize planned services to their optimal benefit.
- Attends and participates regularly in supervisory meetings, staff meetings and case conferences.
- Completes promptly, timely and accurately all required reports including current case recordings and other necessary statistical and narrative reports.
- Will assist clients with money management issues. This may include setting up a budget, paying bills and assisting with money decisions.
- Performs other related duties as required.
QUALIFICATIONS:
- B.A./B.S. from accredited university in social work-related field.
- Ohio L.S.W. or L.P.C. licensure required.
- 2 years of experience working with seniors a plus.
- Ability to work as a member of multidisciplinary team with a solid understanding of the aging process along with knowledge of individual, family and social systems.
- A willingness to try new approaches and good interviewing skills are a must.
- Must be computer literate and have strong verbal and written communication skills.
- Must pass a pre-employment drug/alcohol screen, to include THC/marijuana, and background check.

- Must be in general good health and demonstrate emotional stability.
- Have the ability to cope with the mental and emotional stress of the position.
- Ability to ascend and descend stairs.
- Stand, bend, lift, move and sit for extended periods of time during working hours.
- Ability to make independent home visits throughout service area using visual observation skills to assess and evaluate client’s personal well-being and living conditions.
