Responsibilities

The Manager of Care Management is responsible for the development and management of the infrastructures that support the effective placement, transition, ongoing monitoring of services, the final disposition of every patient treated at University Hospital, and the 24-hour accountability of the Center for Emergency Care.

  • Manages, supervises, and monitors activities of supervisors, Masters prepared social workers, support staff; and social work students in assigned areas plan, coordinate, and implement programmatic changes that enhance improved patient care.
  • Develops, manages, and evaluates programs and services. Participate in activities that monitor, refine, and promote efficient patient throughput.
  • Removes barriers that impact patient care; escalate system issues up through Director, VPs and the CEO. Promote inter and intra-department communications.
  • Establishes and maintains a working environment that supports the RITE Values. Motivate staff to achieve the highest level of professional and personal development.
  • Develops and implements education programs for social workers and other multidisciplinary hospital staff; provide instruction and consultation to medical students and residents. Coordinate social work student internships.
  • Develops and maintains liaison relationships with community agencies, extended care facilities and funeral homes. Provide direct patient care services as an integral part of the escalation process.
  • Participates in weekly patient and associate rounding activities and prepare summary reports.
  • Participates in hospital quality improvement teams, committees, and special projects. Actively participate in the department Continuous Quality Improvement (CQI) program.
  • Develops methods and measuring devices that educate staff and improve the quality of documentation.
  • Demonstrates competency in addressing the unique age-related physical, psychosocial and social needs of patients across the life span.
  • Provides social work supervision to staff to promote patient/family satisfaction. Troubleshoot and mediate patient/family issues with social workers.
  • Provides consultation and training to medical staff and other healthcare professionals on psychosocial issues regarding illness and barriers to maximizing improved health status.
  • Collaborates with co-workers and management to ensure the on-going development of the Social Work Department

Qualifications

  • Master's degree preferred.
  • MSW, LISW- S Required.
  • Licensed Master prepared Social Worker Certification in Case Management strongly preferred.
  • Five (5) years combination of clinical nursing and/or social work, care management, and leadership experience in an acute care setting is expected.
  • Experience in level of care management and discharge planning is essential.
  • Third party administration also preferred.
