Job Description

Mission: Deliver a holistic, integrated continuum of learning and care to equip our at-risk citizens to step up to their highest potential. 

Position Title: Care Coordinator 

Accountability: Care Coordinator reports to Director of Care Continuum  

Position Function & Principal Activities: The Care Coordinator is responsible for the daily program oversight related to the care aspects of our Candidates & Members journeys through our program.  

Program Implementation: 

  • Coordinate and help improve a consistent and systematic method to serve LLC Candidates and Members using customized pathways of services through defined needs and actions.  
  • Aid in the tracking process of Candidates and the various Candidate Support meetings. 
  • Assess and fulfill the immediate care needs of all current Candidates and Members and make referrals to partner agencies, including but not limited to scheduling and coordinating with partners, as well as Peer Support Specialist.  
  • Aid in the development and recruitment of Care Continuum Partners.  
  • Support Candidates and Members with problem solving, decision making and goal setting.  
  • Barrier Removal- Meet individually with enrolled Candidates and assist in Care service offering for barrier removal identified in Enrollment Risk Assessment. 
  • Assist and lend support to the Director of Care Continuum in accessing escalated barrier removal on a case-by-case basis, as needed.  
  • Make care calls on a weekly basis.  
  • Aid in the coordination, management and improvement of the Continuum of Care programs and grants that support the care continuum and pathways of service activities. 
  • Aid in the tracking and analyzing of the number of Care Offering provided.  
  • Complete monthly reports for various care related grants.  
  • Conduct weekly and monthly Care Closet reviews and aid in the ongoing management and stewardship of all care resources to ensure a consistent and available supply of products to meet the needs of our Candidates and Members. 
  • Aid in the implementation and monitor of Recovery Community Center and KORE funded programming, including but not limited to Saturday programming.  
  • Lead, plan and execute Saturday Recovery Community Center programming. 
  • Assist in the development and implementation of recovery social events and manage events in a Recovery Community Center calendar of events.  
  • Facilitate various recovery support groups as needed. 
  • Assist with implementation of the LLC Rewards Incentive/Dignity Dollars program. 
  • Develop and maintain relationships with community and residential partners and execute Care Connect events monthly. 
  • Coordinate and maintain the integrity of Orientation program materials and contribute to continuous program improvements.  
  • Sign Candidates up for Care Offerings during Orientation. 
  • Serve as back up facilitator. 
  • Other duties as assigned. 

Qualifications: 

The ideal candidate will possess most of the following qualities 

  • Preferred education of a bachelor’s degree in social work, Psychology, Criminology or related field, or equivalent experience. 
  • Public speaking and strong communication skills, experience working with vulnerable populations including individuals living in poverty, ability to project a caring demeanor, motivate individuals, and maintain high expectations for accountability. 
  • Excellent oral and written communication skills. 
  • 2-4 years’ experience coordinating program.
